Simple Ways to Include QCH in Your Plans

1. Bequests (Wills & Trusts)

You can designate Queens Community House as a beneficiary of your will or living trust.
Sample language for your attorney:

“I give [dollar amount / percentage of estate / specific asset] to Queens Community House, a nonprofit organization located at 108-25 62nd Drive, Forest Hills, NY 11375, Tax ID: 11-2375583 to be used for its general purposes.”

2. Beneficiary Designations

An easy way to make a gift is by naming QCH as a beneficiary of your:

• Retirement plan (IRA, 401k, 403b)
• Life insurance policy
• Bank or brokerage accounts
